The APRN did not properly conduct or document a comprehensive physical assessment. The physician also failed to order diabetic foot monitoring, which is part of the facility’s protocol.

The resident’s wound worsened over time. The eventual result was that the toe needed to be amputated, followed by a transfemoral amputation.

As a result of this issue, Touchpoints at Manchester was fined $1,530. This was considered a Class B violation.
