Nursing Home Violations

Despite the positive features of this facility, there are some negative ones to keep in mind. In particular, note that the facility was subject to a 2017 fine for violating the Connecticut General Statutes and Public Health Code.

In early 2017, a resident with dementia and cognitive impairment issues sustained a fall while being transferred with the use of a medical lift. The resident remained on the floor until 911 arrived and transported the resident to the emergency department.

During the transfer in a Hoyer lift, a sling came undone, and the resident began to slide out of the lift and to the floor. A staff member assisting with the transfer admits that while they initially connected the sling, they did not recheck the connection before transfer as per protocol. Furthermore, the resident who was injured says that they asked the staff members if they had rechecked the connection before transfer. The staff members said they had not and proceeded with the transfer.

This resulted in a 3-4 fall for the resident. They sustained several broken ribs and needed to have a chest tube inserted.

The issue was investigated in 2017 and determined a Class B violation of the Regulations of Connecticut State Agencies. Salmon Brook Center received a fine of $1,740.
