Violation description: The Director of Nursing refused to send a resident to the hospital when the doctor instructed two nurses to take them. Because the resident was not taken to the hospital for hours after instructed, the resident suffered a severe decline in functioning and needed emergency surgery and was placed in hospice.

Violation description: A nursing assistant who was in charge of supervising residents outside in the designated smoking area failed to see a resident walk away from the designated spot and into the parking lot.

Date of violation: 2/5/16

Type of violation: Class B health code violation

Violation description: A resident who was prone to falls suffered a serious fall and staff was ordered that this resident be placed on 15-minute watch in order to minimize the risk for falling. The 15-minute watch was never recorded, and no one was notified other than to watch the resident more than usual. The resident sustained 22 falls within the three months subsequent to being placed on 15-minute watch, but no staff followed this procedure because it was never reported.

Cassena Care at Norwalk offers many different types of resources and services, but there are a few main types of care to consider. This facility offers 24-hour skilled nursing care, short term rehabilitation, and long term rehabilitation.

24-hour nursing care is available for those who can no longer take care of themselves and need intensive care. Nurses and other staff members work with residents to establish individualized care plans. These plans ensure that each resident gets the individual attention that they need.
