Before choosing Bloomfield Center for Nursing and Rehabilitation, it is important to understand that they received a violation for a string of issues that have taken place in the past few years.
In 2016, a resident was found unresponsive by staff members. Staff members did not follow protocol, which would have been to stay with the resident, check the resident’s vitals, and immediately start CPR or administer other basic life support services. The staff members contacted emergency services but did not try to revive or help the resident while ambulance services were en route. Emergency services were able to administer CPR, but the staff members did not follow protocol by simply contacting emergency services.
Another resident was readmitted to the facility in early 2017 after sustaining a fall and breaking their femur. The resident was to wear a brace on their leg to immobilize the fracture, but instructions were given to remove the brace each day in order to clean the monitor the leg. These instructions were not written down and were not carried out. As a result, the resident developed pressure sores on their leg.
Another incident occurred in late 2016. Another resident who needed assistance turning over in bed was being assisted by a staff member when the resident’s leg started sliding of the bed. Due to the resident’s obesity, this sliding pulled the resident off the bed and the staff member was not strong enough to stop it. The resident slid to the floor on their knees and sustained a fracture in their knee. It was determined that the staff member had the resident positioned too close to the edge of the bed and that this contributed to the incident.
In late 2015 there was another fall incident with another resident. The resident required the help of two people to be transferred from the bed, toilet, shower, etc. One staff member attempted to transfer the resident from the shower (instead of the required two) and a fall occurred. The resident sustained a humeral fracture as a result of the fall.
These issues combined for Class B violations of the Public Health Code. Bloomfield Center for Nursing and Rehabilitation was fined $3,000 as a result.
Finally, another violation occurred in 2017. A resident who gets insulin each morning was scheduled to have a procedure done. As a result, a doctor ordered that the resident not eat or receive medication on the morning of the surgery. Food was not administered, but insulin was. This caused a lethargic response in the resident and the surgical procedure had to be postponed.
This issue resulted in a Class B violation and a $1,740 fine.
