In 2018, Bel-Air Manor Nursing and Rehabilitation received a violation for an incident with a resident. A resident required the help of two people to get in and out of beds/chairs due to their history of falls. Transfers must also occur with the use of a mechanical lift. In 2017, there was an incident in which the resident fell off of the mechanical lift during a transfer. This resulted in left shoulder pain and it was ultimately determined that the shoulder was dislocated. The fall happened because the resident slipped out of the sling on the mechanical lift. The staff member borrowed a sling from another resident, which ended up being too big.

This incident resulted in a fine to the facility of $3,060.
