Drawbacks of the Facility
Even though Apple Rehab Rocky Hill has many great features and opportunities for the elderly, it is not without issues. The facility received a violation in 2018 for a situation that transpired there in early 2018.
A resident of the facility was being monitored for heart and lung issues. Certain medications were ordered once for the resident to combat high potassium levels. A staff member later made an error in recording the resident’s potassium levels. If staff had been correctly informed of the resident’s potassium levels, they would have sent the resident to the Emergency Room.
This resident ultimately passed away, however, the doctor claims that they did not feel that incorrectly recording the potassium levels contributed to death.
As a result of this issue, Apple Rehab Rocky Hill was fined $1,410. They were issued a Class B violation of Regulations of Connecticut State Agencies.
This facility received another fine of $3,000 for several incidents that transpired with residents.
One resident was at risk for developing pressure ulcers, and certain preventative treatments were to be followed, such as offloading the heels and using a special mattress. Offloading was improperly done on at least one occasion.
With another resident, pressure ulcers were a concern and preventative options including the use of a special mattress. At one point, staff members noticed that the resident did not have the special mattress and their heels were not offloaded. A wound consultant noticed a pressure ulcer formed at some point in the facility.
Yet another resident developed a pressure ulcer. There was not documentation to note revisions to the treatment plan.
Another resident was found to be at risk for pressure ulcers, but their care plan failed to document preventative measures to take, such as offloading the heels. The special bed was also not set to the right setting for the resident’s weight. A pressure ulcer formed for this resident.
Yet another resident was noted to be at risk for pressure ulcers. They were to have their heels offloaded at all times, but a wound specialist found that this was not the case for the resident.
In another situation, a resident who was to have their heels offloaded to prevent leg issues was found without this treatment. A staff member also failed to dress a wound on the coccyx.
Finally, another resident was found to be at risk for bleeding. They were put on medication to treat this issue. This resident received medications in error. One medication was discontinued and the resident was to switch to another. However, the second medication was delayed, so they were delayed in taking it. The staff members failed to document why they didn’t contact the pharmacy to inquire about the delay.
