Be Aware of Violations

Even though this care facility has many useful amenities, services, and resources for the elderly, there are drawbacks to this nursing home. One important drawback to consider is a violation of Connecticut General Statutes and Regulations for Connecticut State Agencies.

Apple Rehab Avon received a violation in 2018 due to an incident with a resident. One resident was noted to be at risk for pressure ulcers, also known as bedsores. These usually form when a person stays in one position for too long. A preventative treatment of off loading the resident’s heels was prescribed, but it was not documented that this treatment was followed.

The resident was later incorrectly identified as being at low risk for pressure ulcers. The resident eventually complained of toe/heel pain and was supposed to be checked every week, though there isn’t documentation that checks were administered.

Body audits, weight check ins, and checks for risk of pressure ulcers were not completed. Issues with bedsores as well as weight issues were not properly monitored, documented, and treated. As a result, the facility was fined $4,520.

A separate violation was issued in the same year for different issues. First, a resident was overheard making comments that they wanted to kill themselves. Even though a staff member reported these comments, the report did not mention that the resident should be closely watched. Protocol was not followed in terms of who to report these comments to and how to proceed.

In another situation, a resident crashed their electric wheelchair into a gait in the parking lot of the facility. The resident threatened staff members with scissors and the police were contacted. However, a report of the incident failed to mention that the resident was placed on one-to-one observation. Certain staff members were also not notified of the incident.

These issues constituted a fine of $1,330.
